Email Order Form (Cake Maker's Version)
An A4 form that you receive as a download (no physical product mailed out). You can then email this form to customers to record their order details. Alternatively, you can print the form out and fill it in by hand.
Each of the boxes are 'clickable' or can be free-typed. Your customer can click to say how many cupcakes they'd like, and then type in their choice of flavour or topping.

No changes to the format are allowed on this product. If you require a more tailored form, please contact us for a quote.
Returns & exchanges
I gladly accept returns and exchanges.
Please contact me within 14 days of delivery and dispatch items back within 30 days of delivery.
Buyers are responsible for return postage costs. If the item is not returned in its original condition, the buyer is responsible for any loss in value.
Please note that once a proof has been agreed on a custom or personalised item, your order will be processed and cannot be cancelled at this point.

Because of the nature of these items, unless they arrive damaged or defective, I can't accept returns for:

When you order this personalised product we will email a proof to you as soon as possible. Please check your email junk box if you haven't received a proof.

This product is a DOWNLOAD only. This means that you will receive it by email. You will NOT receive a physical copy of the form.
